David Oliver: No more broken promises.

چکیده

In clinical practice, trust is paramount. We trust team mates to know their job and to hand over well. Patients and families need to trust that we’re competent, compassionate, and acting in good faith. But sometimes, in trying to reassure patients and with the best of intentions, we make promises we shouldn’t. And broken promises damage trust and confidence. Some promises concern what we as individuals or teams will do for patients. For example, “We’ll do our best to get you as well and as close to your usual abilities as we can.” “We will refer you for this test or to that team.” Expert, evidenced, or educated prognostications about how much better or worse a patient might get or what a test or treatment might achieve—these are, partly, promises: “This treatment might improve your pain control and help your mobility, but in your case there’s only a 50/50 chance.”

Worth Keeping but Not Exceeding: Asymmetric Consequences of Breaking Versus Exceeding Promises

Promises are social contracts that can be broken, kept, or exceeded. Breaking one’s promise is evaluated more negatively than keeping one’s promise. Does expending more effort to exceed a promise lead to equivalently more positive evaluations? Although linear in their outcomes, we expected an asymmetry in evaluations of broken, kept, and exceeded promises. Whereas breaking one’s promise is obvi...

Others have life changing events such as stroke or hip fracture, bringing new disability. Some will tran...

Promises and lies: can observers detect deception in written messages

We design a laboratory experiment to examine predictions of trustworthiness in a novel three-person trust game. We investigate whether and why observers of the game can predict the trustworthiness of hand-written communications. Observers report their perception of the trustworthiness of messages, and make predictions about the senders' behavior. Using observers' decisions, we are able to class...

Politics and a Broken Promise: Why People Are Losing Their Physicians in Post-aca America.

THE LEVEL OF EXCITEMENT THAT MANY AMERicans had about the benefits of the Affordable Care Act [ACA] has not always been echoed by the realities of health care in our emerging delivery system. In particular, the often-repeated pledges that citizens would lose neither their insurance nor their doctor have left many feeling betrayed and abused by broken promises. Although it is true that President...
